Unsure as to whether it was ever signed. MD 686 is the only current route number assigned to be totally within the limits of Baltimore City (which SHA doesn't maintain.) Anne Arundel's portion, which ran from the city line down to Tungston Street, was dropped in 1983.

Unmarked.

Some sort of station on the B&O Railroad? It's on Beltsville Agricultural Research Center property according to my maps. Only appears on my 1952 map of the DC suburbs, but was still on the books in 1995, but it's only 0.1 mile long. It's just across the tracks from US 1. Go east on 212 from US 1...it's the first right turn after the r/r bridge.

This road is only named as "5105-5129" on the green street signs, I guess house numbering for Powder Mill Road. It ends after about 500 feet at a gate into US Government property. The pavement is in such a state of disrepair that it could be mistaken for a driveway. If 694 someday gets decommissioned, it will be a driveway.

The east side of the Baltimore Beltway, east of I-95, is still officially designated MD 695. The official I-695 would have been lengthened to meet with I-97 at the time that was designated. Signed as I-695 at least since 1990. Still a few remnants of the MD designation around, if you know where to look.

MD 695, the non-Interstate that is signed as one, is the opposite situation of US 50, which is officially I-595 but not signed as such.

NHS: Entire length, though not in automatically by being a regular Interstate.

Many maps show the outer (Baltimore County) portion of Broening Highway, as MD 695. This is actually MD 695A, and the unsuffixed Beltway remains on the books.
